Combined Connections-United Vision Plan (UVP)  

We have a new Combined Club offered through Combined Connections called United Vision Plan. We will be assisting with benefit questions, billing questions and cancellations (attempt a soft save). If a member needs help with the UVP website, their password, or their eyewear order, direct the member to UVP at 1-844-244-1184 or email info@UnitedVisionPlan.com.
· Cost-$1.00 for the first 30 days, then $5.95 each month after.
· Membership materials-there will not be a paper kit sent to the members, they will only receive e-kits. Please note, if the member resides in the state of Illinois, we will send both an email and a paper kit.

·Member inquiring about their UVP email or password, direct them to UVP's customer support (1-844-244-1184 / service@unitedvisionplan.com).
·When a member joins through the Combined Connections website, they will receive a Confirmation Page, a Welcome E-mail and a 2nd Welcome E-mail with their login information.

The member will first receive a Welcome email confirming their purchase, the member will then receive a second email from UVP to the email address used to enroll with a temporary password within 1 hour of enrollment.
